I'm transitioning to the self limited care and accommodation and the staff at self limited are brilliant and outstanding and the staff and service users are really nice and kind and polite and when im going to the house 14 park view I'm greeted and cared for and that I've got my own bedroom and facilities and the managers and support workers take me on section 17 community leaves and outings and that the staff and service users engage and talk to me and make me feel at home and im going to the everyone active gym / evolution gym and I'm coming back off leave and having a shower in my bedroom and in my bedroom I have two bay windows and the 24 hours support when I'm moving into the self limited and when I get my overnights from the ministry of justice however the self limited team have said the my community leaves and outings will be with my singleton male or female and that I will be dropped off and picked up from the secure outreach transitioning teams and that im asked to complete my leaves planning whilst in the community areas and that I'm doing my own cooking and cleaning my own bedroom and if I have any problems I will talk to the support workers and last resort talking to the managers and that im living in shared accommodation with other male service users whom having learning disabilities and who have escorted and unescorted and that I might spend time in my bedroom or in the shared areas and or lounge and move my personal belongings to my new home 14 park view and that I will respect staff and other service users and I'm hoping to find the outcome from the ministry of justice about my overnights stay at the self limited care and there accommodation around the 12.02.2026 on the Thursday and that my new doctor and or consultant is Dr Philipps a female consultant from the Northgate park hospital and there won't be any changes to the discharging plan and that im visiting the self limited and that the team and my care package is so outstanding and that the team at the self limited explain everything to me and spend loads of time with me and encourage me to do things for myself and to get to know there staff and service users and about getting the house to become my permeant residence and that I can have my down time and me time and personal time and that I'm able to talk to the self team and promoting independence and money and budgeting skills and good mental health and understanding and attending the mens physical health screenings and the sleeping well and medication and risk management and community and relationships and about tolerating others and people with disabilities and about the shared areas and about the environment and boundaries and with having structure and life and living skills and as much as realities skills and doing as much as possible for myself and also caring for people with the palliative end of life care and treatment and with supporting staff and service users about care and treatment and about adversity and family conduct disorders and with behavioural problems and about having self control despite myself sometimes letting my feelings show meaning I'm having to leave all of the areas and with normally talking about problems and using sensory fidgety toys and using distractions meaning walking and or gymming and singing and laughing and smiling and dancing and psychology and section 17 community leaves and outings and getting to know myself and my body and having time alone television and music and sitting the dark and rocking back wards and forwards on my bedroom chair and using my mindfulness and colouring books and group chatting and having conversations and having a cuppa and having a laugh and a bit of a giggle and visiting localised areas with the self team and doing shopping at the Tesco and herons and gadget shops and going to the cafes meeting different people and going for a walk at the river and park and with visiting different areas and that the self limited managers and there support workers are good at what they do and supporting disabilities and there service users by giving people care and accommodation and giving them life and understanding and a roof over there heads and activities around care and treatment and with having the staff teams to talk too and regular food and drink and there medication and there own space and bedroom and community outings and leaves and seeing there family and friends and the self limited accommodation and there care very very very outstanding and brilliant care and support around health and disabilities and the best outcome around palliative end of life care and treatment and the staff are always there for me and everybody else and that where there for each other in time and need.
